PRESCRIPTIONS

Prescription renewals should be taken care of at the time of office visits. If your prescription should run out before your next appointment we prefer that you call our prescription line at 475-9697 (at least 48 hours in advance) and arrange to come by to pick up the written prescription. This is the best way to avoid errors in prescription numbers and strength. Prescriptions can be called in on emergent circumstances or if stopping at the office is inconvenient. For all renewals please have the full name of the medicine, strength and frequency of dosing, as well as the pharmacy phone number.

FINANCIAL POLICY

We participate with: CDPHP, MVP, BC, BS, Medicare, NYS Empire Plan, Senior Blue, Medicare Choice, GHI-HMO (only), Aetna, United Healthcare, Nova and many other insurance plans.

All co-payments are due at the time of service. If a co-pay is not made at the time of the visit an additional $ 8.00 will be charged.

Returned check charge $ 25.00

Patients who do not have insurance, or who are covered by a plan we do not participate with, will be provided with a statement to submit to their insurance. Payment is due at the time of service unless other arrangements have been made in advance with the billing staff. For your convenience, we accept American Express, MasterCard and VISA.

CONFIDENTIALITY

We maintain confidentiality of all medical information unless a patient specifically authorizes release. No information is accessible to anyone who is not authorized to obtain such information unless the patient or his/her legal representative has consented, in writing, to such release of information. Once a child reaches the age of 18 we cannot discuss any medical information with parents without a written consent.

HOSPITALS

Dr. Drew, Dr. Piplani and Dr. Sullivan have admitting privileges at St. Peter's Hospital.
